增强T2 FLAIR序列及延迟8分钟T1WI序列在脑转移瘤中的诊断价值

目的:评估增强T2 FLAIR序列和延迟8分钟T1WI序列在脑转移瘤诊断中的价值。方法:回顾性分析安阳市肿瘤医院2022年3月至2023年4月收集的31例脑转移瘤患者的磁共振数据,包括T2 FLAIR、增强T2 FLAIR、增强T1WI和延迟8分钟增强T1WI序列。受试者均使用同一台1.5T飞利浦MR机器扫描,并且均以2.0 mL/s的注射速度经肘静脉推注对比剂(Gd-DTPA)(剂量0.1 mmol/kg)。结果:31例中患者中T2 FLAIR检出83个病灶,显示率为31.8%;增强T2 FLAIR检出247个病灶,显示率为94.64%,早期T1WI增强检出76个病灶,显示率为29.12%,注射对比剂后8分钟T1WI增强检出199个病灶,显示率为76.25%,联合使用增强T2 FLAIR序列和延迟8 min T1WI序列可提高诊断准确率至98.08%。并且两位影像医师均认为增强T2 FLAIR序列中脑转移瘤图像清晰比例最高,诊断一致性良好(P<0.05)。脑转移瘤T2 FLAIR的CBR、CNR及SNR值均小于8 min增强T1WI,同时也小于增强T2 FLAIR,差异具有统计学意义,T2 FLAIR与早期增强T1WI差异无统计学意义。结论:增强T2 FLAIR序列和延迟8 min T1WI序列的联合使用可以提高脑转移病灶的检出率及诊断准确性。

Abstract:

Objective: To assess the value of enhanced T2 FLAIR sequences and delayed 8-minute T1 WI sequences in the diagnosis of brain metastases. Methods:Magnetic resonance data including T2 FLAIR, enhanced T2 FLAIR, enhanced T1 WI, and delayed 8-minute enhanced T1 WI sequences were retrospectively analyzed in 31 patients with brain metastatic tumors collected from March 2022 to April2023 in Anyang Tumor Hospital. The enhancement characteristics of the metastatic sites in terms of size and morphology were analysed on the basis of plain scanning using cephalometric scanning sequences with the same parameters in all patients: T2 FLAIR, enhanced T2 FLAIR, enhanced T1 WI, and enhanced T1 WI with 8-minute delay. Subjects were scanned on the same 1.5 Philips MR machine and all were pushed with contrast agent(Gd-DTPA) via an elbow vein at an injection rate of 2.0 mL/s(dose 0.1 mmol/kg). Results: T2 FLAIR detected 83 lesions in 31 patients with a display rate of 31.8%, enhanced T2 FLAIR detected 247 lesions with a display rate of 94.64%, early T1 WI enhancement detected 76 lesions with a display rate of 29.12%, and 8-minute T1 WI enhancement after injection of contrast agent detected199 lesions with a display rate of 76.25%, and the combination of enhanced T2 FLAIR sequences and delayed 8-minute T1 WI sequences improved the diagnostic accuracy to 98.08%. And both imaging physicians agreed that the highest percentage of brain metastases images were clear in the enhanced T2 FLAIR sequences, with good diagnostic consistency(P<0.05). The CBR, CNR and SNR values of T2 FLAIR of brain metastases were smaller than those of 8-min enhanced T1 WI and also smaller than those of enhanced T2 FLAIR, and the difference was statistically significant, and the difference between T2 FLAIR and early enhanced T1 WI was not statistically significant. Conclusion: The combination of enhanced T2 FLAIR sequences and delayed 8-minute T1 WI sequences can improve the detection rate and diagnostic accuracy of brain metastatic lesions.
